GP services
In response to the COVID 19 pandemic we continue to take measures to protect our patients and staff. This has changed the way we care for you, but has not reduced your access to our advice, diagnosis and care.
Our doctors are consulting five days per week and are providing a mix of face-to face and tele-health consultations. Please call the clinic first to talk to our friendly staff about making an appointment. If you are experiencing flu like symptoms, you will be asked some questions and provided information on what to do next. This will help protect the health and safety of you and others.
Wellness
It is our priority at Inclusive Health and Wellness Hub to provide a safe and supportive environment for our vulnerable client base and in so doing to support the health of the greater community. In line with current government recommendations we are delighted to announce a return of some of our wellness services, including acupuncture, mythotherapy and Zen Thai massage. Some services will be delivered a little differently for now, however as restrictions lift we will adjust and keep you informed. To access our wellness service call the clinic to secure an appointment.

We encourage all our valued participants to continue doing things they can to stay well. May we recommend nourishing meals, adequate rest and a gentle daily movement practice such as stretching, walking, qi gong or yoga.

Good oral health is an essential part of overall health and wellbeing.
Our dedicated team of Tzu Chi volunteer dentists, dental therapists (people who treat common dental problems) and dental prosthetists (people who make dentures) are committed to delivering quality dental services.
Who can use the service?
Dental services are provided for specific population groups who meet eligibility criteria and referrals can be made from services who are supporting people experiencing disadvantage. To enquire about eligibility, please contact the Inclusive Health and Wellness Hub. Homefront Outreach Team is collocated at the Inclusive Health and Wellness Hub and delivers outreach services that connect people with clinical care, community support and housing by providing integrated care.
Homefront is a collaboration between Micah Projects, Princess Alexandra (PAH) and Mater Hospitals and primary care providers. Beneficiaries are people experiencing poor health due to a combination of homelessness/vulnerable housing, domestic violence, chronic health conditions, disability and social isolation.
The outcomes will be:
- improved access to clinical care, social support and housing
- better outcomes for individuals
- reduced presentations to Emergency Departments (EDs), and
- less unnecessary hospitalisations.
What is Homefront?
Homefront is funded by Queensland Health to improve the outcomes of people with complex health needs who present frequently at Hospital Emergency Departments.
Homefront works with the PAH and Mater Hospital Brisbane, and community-based services to deliver integrated care in the immediate PAH catchment.
How does Homefront work?
Micah Projects and the Inclusive Health and Wellness Hub have implemented and will evaluate an integrated model of care delivered by a multidisciplinary team. The team provides outreach services to vulnerable people in the community who are living with multiple and complex health and social needs.
Integral to the program’s success will be collaboration between the Hospital ED stakeholders and the Homefront multidisciplinary outreach team. Homefront supports people to effectively engage with community-based services and thereby reduce hospital attendance/dependence.
In addition to referrals from Hospital EDs, Homefront also works with local GPs and homelessness services to ensure as many people as possible benefit from the service. People can also self-refer.
Homefront deliverables
Homefront’s primary focus is health and wellbeing and the deliverables are:
- Improved health and wellbeing of vulnerable populations through integrated care.
- A reduction in ED presentations by people in the target group (vulnerable members of the community who may invariably have multiple health and housing issues. Many of the participants may be experiencing homelessness, utilising social housing, have experienced domestic violence and/or be experiencing difficult family circumstances.
- Provide an integrated care response where one does not currently exist or exists and could be more effectively managed.
- Provide a continuum of care and referral point primarily from hospital EDs and hospital departments, through effective discharge planning. This will reduce unnecessary re-presentations.

The Clinical Nurse Partnerships – Domestic Violence Project has a dedicated Clinical Nurse to support women to meet their health needs after experiencing domestic violence. The domestic violence initiative aims to improve outcomes for these women accessing primary healthcare services.
What is the Clinical Nurse Partnership – Domestic Violence project?
The Clinical Nurse Partnerships – Domestic Violence project has employed a Clinical Nurse to provide direct nursing care, immediate health assessment, education, care coordination and navigation through the health system. This is provided to a limited number of women who have experienced domestic violence in addressing the health impacts of physical, psychological and sexual violence. The improved outcomes achieved through this innovative project will be used to identify gaps and possible solutions to improve services to women who have experienced domestic violence. This project is funded by St Vincent’s Health Australia and operates across Monday to Friday (8:00am – 4:00pm)
Clinical Nurse Partnership – Domestic Violence project deliverables
The primary focus of the Clinical Nurse Partnerships - Domestic Violence project is health and wellbeing and the deliverables are:
- Improved health for women experiencing domestic violence.
- Provide a continuum of care and referral point primarily from hospitals, through effective discharge planning.
- Increased access to healthcare services through timely assessment and direct nursing care for women experiencing domestic violence.
- Assistance to navigate the health system.
What is the Inclusive Health and Wellness Hub?
The Clinical Nurse works in partnership with the Inclusive Health and Wellness Hub. The Hub is based in South Brisbane (Shop 2, 15 Hope Street) and is a primary care centre offering general practitioners, nurses, allied health practitioners, dentists, alternative therapies and community / psychosocial support with strong linkages to social and community services. The Hub delivers care to vulnerable people and is an excellent referral point for Hospital’s when people present who would be better suited to high quality primary care.
Referral Criteria
The primary inclusion criteria are:
- Women experiencing domestic violence with related health issues, requiring effective clinical management in the community.
